Information

Dog Requirements

  1. Must be fully vaccinated and current, as required by the state and recommended by the veterinarian.
  2. Must be licensed with the city or county.
  3. Have a safe and strong leash
  4. Have a flat buckle collar with ID tags (in addition to any training/walking devices)
  5. Friendly with unfamiliar people (if your dog is reactive to unfamiliar people/animals/dogs, a behavior evaluation is required before service begins)
  6. Have an appropriate muzzle, if required.

House Keys

Two copies of your house key are required. One copy is for daily use - the other is kept in a secure location as a back up in case of emergency. All keys are coded and do not have your name or address written on them. If you do not have two copies of your key ready for us, we can make a copy of your key for you, for a $5 charge.

Leaving a key on file

This allows for the availability of short notice visits, and is required for recurring visits. Methods of key return are listed on the contract if you do not wish to keep a key on file for future needs.

House codes/Garage Door opener

An opener or code will be accepted if this is the preferred method of entry. However, a physical key is required in case of power outage or malfunctioning equipment.

Off-leash

Your dog will never be let off leash during walks for any reason, except for medical emergency. It is State law to have dogs on leash when off the owners property.

Please read all of the following, and then choose from the buttons below to submit your contract.

Terms and Conditions

  1. The term of this contract may be for daily/weekly/bi-weekly, or another schedule to be determined at your initial in-home consultation.
  2. The fee for a walk is $15 per 30 minute session. $5 per additional dog will apply, and additional fees may apply if the dog is reactive or aggressive toward dogs, other animals, or people.
  3. For pet sitting: There is a 3-walk per-day minimum if owners are out of town for more than one day. If an overnight stay is desired and we are available to accomodate, the fee is $60, and includes the evening visit, the overnight stay, and the morning visit.
  4. For insurance purposes, if owners are out of town, Mutt Magic Training, Inc. (DBA The Crate Escape) is the only service allowed to enter your home to handle your animals. Neighbor and/or family assistance is prohibited during times when Mutt Magic is caring for your pets, unless an exception is made and agreed to in writing.
  5. Payment is due at the time of service, and must include any additional fees or charges. Checks may be made payable to Mutt Magic, and mailed to the address on the invoice or left for pickup at the time of service. Payments made by credit card are accepted through PayPal for an additional 3% fee (send to: aja@muttmagic.com) Client understands this contract and takes responsibility for prompt payment of fees. Client authorizes this contract to be valid approval for future services of any purpose provided by this contract, permitting Mutt Magic Training, Inc (DBA The Crate Escape) to accept telephone or email reservations for service and enter premises without any additional signed contracts or written authorization.
  6. Mutt Magic Training, Inc. (DBA The Crate Escape) is authorized to walk the dog as outlined in this contract.
  7. Mutt Magic Training, Inc. (DBA The Crate Escape) is authorized to seek any medical care if deemed necessary with medical and/or emergency treatment (excluding euthanasia) as recommended by a veterinarian. Client agrees to reimburse Mutt Magic Training, Inc. (DBA The Crate Escape) for any expenses incurred attending to this need.
  8. In the event of a personal emergency or illness of a scheduled dog walker, client will be notified as promptly as possible. Walks may be rescheduled, or client may be compensated the cost of missed walks as a result of this.
  9. The client is responsible for any costs/payments due to bites. If a bite occurs, Mutt Magic Training, Inc. (DBA The Crate Escape) is responsible for reporting it to authorities.
  10. All dogs are required to be fully vaccinated as required by law, and as recommended by the client's veterinarian. Dogs are also required to be licensed by their city.
  11. Mutt Magic Training, Inc. (DBA The Crate Escape) reserves the right to terminate this contract at any time before or during this term.
  12. Client understands this contract is for walking services only, and does not include training. (An additional services agreement will be provided if you need or choose Walk & Train services)

Cancellation Policy

A minimum of 24 hours cancellation notice is required, except in the case of extreme emergency. Cancellations occurring with less than 24 hours notice will be charged the full rate. If a client wishes a total cancellation of all walking dates and services with Mutt Magic Training, Inc. (DBA The Crate Escape), please make these wishes known in writing.

Inclement Weather Policy

In the best interest of your dog and our staff, walks will not be occurring if:

  1. The local county schools are closed as a result of the weather.
  2. The temperature is above 90 degrees (depending on your dog's age, health, coat, and endurance)

In the event of inclement weather that does not pose a travel risk (rain, heat, etc) the client can agree to a home visit during a regularly scheduled walk. The dog will be given a potty break, and the rest of the time will be spend in-doors. Indoor activities may include, but are not limited to: fetch, tug, recall games, hide and seek with toys or treats

Release of Liability

By clicking the submit button below, I do hereby waive and release Mutt Magic Training, Inc. (DBA The Crate Escape) from any and all liabilities of any natures for the actions of myself, my pet(s), or any other person who accompanies me, or holds a key to my home; except those arising from the negligence or willful misconduct on the part of a Mutt Magic Training, Inc. (DBA The Crate Escape) contractor. Mutt Magic Training, Inc. (DBA The Crate Escape) agrees to provide all services in a kind, humane, reliable, and trustworthy manner. Client agrees to notify Mutt Magic Training, Inc. (DBA The Crate Escape) of any concerns within 24 hours of their return. In case of emergency, inclement weather, or natural disaster, I authorize Mutt Magic Training, Inc. (DBA The Crate Escape) to use reasonable judgment for the care and well being of my pet(s) and/or house.

I understand that Mutt Magic Training, Inc. (DBA The Crate Escape) can terminate this contract if my pet becomes a threat to the safety or health of any Mutt Magic Training, Inc. (DBA The Crate Escape) contractor or the community due to aggressive behavior. Mutt Magic Training, Inc. (DBA The Crate Escape) will contact client if pet(s) display aggressive behaviors or tendencies; wee do specialize in various forms of dog aggression modification, and will do what we can to assist families with special needs dogs. Mutt Magic Training, Inc. (DBA The Crate Escape) is required to report any bites to authorities. I acknowledge I am responsible for medical expenses and damages resulting from an injury caused by my pet(s). Mutt Magic Training, Inc. (DBA The Crate Escape) reserves the right to refuse service to any client, at any time, for any reason.

I attest that all of the above information is true to the best of my knowledge. If anything changes from what is listed, I will inform Mutt Magic Training, Inc. (DBA The Crate Escape) before the next service is scheduled.
